Dalej: „my” albo „Restauracja”, a lokal wskazany wyżej — „Lokal”.

1. Zgoda na publikację

Wyrażamy zgodę na to, aby PLATE SOCIAL-FI SPÓŁKA Z OGRANICZONĄ ODPOWIEDZIALNOŚCIĄ z siedzibą w Stanisława Noakowskiego 10 / 20, Warszawa, 00-666 M.st. Warszawa, Polska („Plate Social”) publikowała informacje o Lokalu:

2. Jakie informacje obejmuje zgoda

3. Skąd pochodzą informacje

Informacje i materiały mogą pochodzić:

4. Logo, zdjęcia i inne materiały

7. Czas obowiązywania i odwołanie zgody

6 ust. 1 lit. b i lit. f RODO, a w zakresie objętym zgodą — na podstawie art. 6 ust. 1 lit. a RODO.

Version 2026-09-17

Restaurant statement: consent to publish venue information and licence

This is an English translation for convenience. The Polish version is binding.

The statement is made by the business running the restaurant, identified in the application form on the Plate Social website (company name, NIP tax number, registered address), in respect of the venue identified in the same form (venue name and address). It is made on the Restaurant's behalf by the applicant named in the form.

Hereinafter: “we” or “the Restaurant”, and the venue identified above — “the Venue”.

1. Consent to publish

We consent to PLATE SOCIAL-FI SPÓŁKA Z OGRANICZONĄ ODPOWIEDZIALNOŚCIĄ, registered office at Stanisława Noakowskiego 10 / 20, 00-666 Warsaw, Poland (“Plate Social”), publishing information about the Venue:

2. What information the consent covers

The consent covers in particular:

This list is not exhaustive. The consent also covers other information about the Venue of a similar nature that we provide later or that Plate Social obtains as described in section 3. Extending the published information to data of this kind does not require a new statement.

3. Where the information comes from

Information and materials may come:

We confirm that Plate Social may publish information from these sources within the scope described in sections 1 and 2.

We undertake to inform Plate Social of changes to information that matters to guests — in particular opening hours, address, offer and prices. Plate Social is not responsible for information being out of date where we have not reported the change.

4. Logo, photos and other materials

We grant Plate Social a free and non-exclusive licence to use the logo, identifiers, photos, descriptions, menu and other materials provided by us or obtained in accordance with section 3, within the scope described in section 1, without territorial limitation, for as long as the consent remains in force.

The licence covers recording and reproducing the materials digitally and making them available online and in the app in such a way that anyone can access them at a place and time of their choosing.

Plate Social may:

5. Our assurances

We declare that:

If a third party brings a claim against Plate Social relating to materials we provided, we will join the proceedings on Plate Social's side and cover reasonable costs and any amounts Plate Social has to pay as a result.

6. Free of charge

The consent and the licence are free of charge. Neither party acquires any claim for payment on this account.

This statement does not oblige Plate Social to list the Venue in the app, on the website or on the map, or to keep it published for any particular period. It is not a cooperation agreement and gives neither party exclusivity.

7. Duration and withdrawal of consent

The consent is given for an indefinite period and remains in force until withdrawn.

We may withdraw it at any time — in whole or in part — by sending a statement to jakub@platesocial.app or in writing to Plate Social's registered office. Plate Social removes the information and materials covered by the withdrawal from the app, website and map within 14 days of receiving it.

We acknowledge that after that period the information may still remain for some time in backups, in search engine and browser caches, and in materials already distributed — for example previously published posts, printouts and press materials. Plate Social does not have full control over this and does not remove such materials retroactively.

8. Personal data

Plate Social is the controller of the personal data of the person signing the statement and of the contact details provided above. The data is processed to present the Venue in the app, on the website and on the map, and for contact on related matters — under Art. 6(1)(b) and (f) GDPR and, to the extent covered by consent, Art. 6(1)(a) GDPR.

We have the right to access the data, have it rectified, erased and its processing restricted, the right to object, and the right to withdraw consent at any time without affecting the lawfulness of processing carried out before withdrawal. Full information on data processing is available at https://platesocial.app/privacy. Contact on personal data matters: jakub@platesocial.app.

9. Final provisions

This statement is governed by Polish law.

The statement may be made in writing or electronically — by returning a signed scan by email, by electronic signature, or by ticking the relevant box when registering the Venue. Each of these forms has the same effect.

Plate Social notice — not part of the statement

How we process personal data from the application form (Art. 13 GDPR)

Controller. PLATE SOCIAL-FI SPÓŁKA Z OGRANICZONĄ ODPOWIEDZIALNOŚCIĄ, ul. Stanisława Noakowskiego 10/20, 00-666 Warsaw, Poland, NIP 7011311776. Data protection contact: jakub@platesocial.app.

What data. The applicant's name, role at the restaurant, email and phone; company and venue details; the date and time the statement was made, IP address and browser information.

Purposes and legal bases.

  • handling the application, publishing the Venue and contacting you about it — Art. 6(1)(b) and (f) GDPR (our legitimate interest is running the app and venue map and communicating with restaurants),
  • keeping a record of the statement and consent, and establishing, pursuing or defending claims — Art. 6(1)(f) GDPR,
  • sending commercial information by email and phone — only if you tick the marketing consent — Art. 6(1)(a) GDPR in conjunction with Art. 398 of the Polish Electronic Communications Law.

Recipients. Service providers processing data on our behalf: website hosting (Vercel Inc.), email delivery (Resend) and email (Google). They may process data outside the European Economic Area — on the basis of the European Commission's EU-US Data Privacy Framework decision or standard contractual clauses.

How long. For as long as the consent to publish remains in force, and then until claims become time-barred (generally 6 years). Marketing data — until you withdraw consent or object.

Your rights. Access, rectification, erasure, restriction of processing, data portability, objection to processing based on legitimate interest, and withdrawal of consent at any time without affecting the lawfulness of prior processing. You may also lodge a complaint with the President of the Personal Data Protection Office (Prezes UODO, ul. Stawki 2, 00-193 Warsaw, Poland).

Do you have to provide data. Providing data is voluntary, but we cannot accept an application without the fields marked as required. We do not make automated decisions or carry out profiling.

After you enter a NIP, company details are fetched from the Ministry of Finance's public register of VAT taxpayers (the “White List”).
